factors affect pain severity and how pain interferes with one's
life. Well-recognized factors that influence treatment outcomes
include the severity of substance abuse, anxiety,
depression, PTSD, and attitude toward improvement.

are in the public domain. They are not intended for diagnostic
purposes. In some cases, "scoring guidelines" are not included,
as the authors recommend discussing the results with your
provider. You are encouraged to complete each
questionnaire and share the results with your providers.
