Schedule an Appointment

Our physicians accept referrals from physicians, social workers, psychologists, rehab programs and other treatment providers. Prospective patients may also contact us directly. Our staff will not schedule an appointment without first speaking directly with a prospective patient; this way we can discuss various treatment options-- helping the patient choose the best treatment. We will refer patients to other providers when we believe this is in the patient's best interest.

In order to accommodate patients, our clinicians do offer evening hours. Patients are urged to review and complete appropriate medical record forms prior to the first visit.
